Introducing the ABA Supervisor-Supervisee Fieldwork Evaluation Forms
Ensure you remain in compliance with the BACB Ethics Code (5.06 and 5.07) and have an objective way to evaluate the supervision experience for both the supervisee and supervisor.
 
There are two forms included in the bundle:
The Supervisor Evaluation Form
This is a 6-page editable document that can be used to evaluate the supervisor’s performance during the fieldwork experience
Behaviorally Anchored Rating Scales (BARS) have been customized for each of the supervisor evaluation sections (scheduling/availability, feedback, supervisor demeanor-behavior, supervision content/skill development, and documentation)
Supervisees can use this to evaluate the supervisor and the effects of supervision
This document can easily be adapted to a Google Form or Microsoft Form for ease of use, and to graph the performance data
